In connection with your use of the Services, we will collect certain contact information from you, such as home address, date of birth, and sex at birth, which we need in order to refer you for a Covid-19 test if you are selected. From your university or employer we may receive your name, email address, phone number, and other information. Such information is collected for the purpose of contacting you with important information regarding the Healthy at Work program and referring you for a Covid-19 test if appropriate. We may share your information with authorized testing sites, health care professionals involved in collection of your biospecimens if you have a test, and clinical laboratories that process collected biospecimens, for purposes related to performing tests.
The Services include features that allow you to complete a screener questionnaire. The information collected in connection with the screener questionnaire may include your age and/or date of birth, demographic information, recent travel history, current health status and any known contact with other individuals who may have been exposed to COVID-19. Such information is collected for the purpose of determining whether you are referred for testing under this program, for determining your overall risk of COVID-19, and for helping to ensure safety at the premises of your employer and/or university. Your questionnaire responses may be shared with your employer and/or university.
In the event that we determine that you are eligible for testing under this program, you may receive certain scheduling information, including the date(s) and times during which you may come to your assigned site for testing. We may also provide such scheduling information (together with your contact information) to authorized testing sites, health care professionals involved in collection of any biospecimens, and clinical laboratories that process collected biospecimens, for purposes related to performing tests. We also collect information from these parties regarding whether the test scheduled for you was conducted.
In connection with your use of the Services, we may ask you to provide certain health insurance information, which may include your policy number, group identification number and, if applicable, the primary insured's name, address, phone number and date of birth. Such health insurance information is collected for the sole purpose of sharing this information with individuals and entities, including healthcare professionals and clinical laboratories, that provide services as part of the testing program, so that insurance can reimburse those parties for the test that is given. If collected, your health insurance information will only be used by third parties for the purpose of submitting claims for reimbursement to your health insurance plans for the services they provide.
The Services include features that allow you to complete a Release of Medical Information Form (the "Authorization"), which may be required in order for you to access certain portions of the Services, such as the Covid-19 test. The Authorization may collect certain information, including your full name, initials and electronic signature. Such information is collected for the purpose of executing the Authorization. Where necessary, we may share an executed copy of the Authorization with authorized testing sites, health care professionals involved in collection of any biospecimens, and clinical laboratories that process collected biospecimens, for purposes related to performing tests and communicating test results.
In the event that you are tested under this program, the Authorization may permit Verily to receive such test results from your testing site, health care professionals, and/or clinical laboratories, as applicable. When collected by Verily, such information may be used for the purpose of communicating your test results to you, sharing the results with your university and/or workplace as applicable, and for purposes of providing and improving services relating to workplace safety, including through the generation of statistics that do not identify you personally. Once information has been shared with your university and/or workplace, Verily cannot control how the information is treated or used. If you have questions about this please consult your university and/or workplace as applicable. The health care professionals and/or clinical laboratories that receive your Results Information may have legal obligations to share that information with public health authorities.
If your employer and/or university allows it, the Services may provide you an option to opt into being contacted about opportunities to participate in health research. If you opt in, your personal information may be used to contact you about opportunities to participate in research.

Google's access to data is strictly limited to the purpose of providing such services to Verily. Your data collected through the Services will never be joined with your data stored in Google products without your explicit permission.
Salesforce: Verily leverages certain technology services from Salesforce.com, Inc., including customer relationship management solutions. Salesforce's access to data is strictly limited to the purpose of providing such services to Verily.
